INCOMING FRESHMAN AND TRANSFER STUDENTS FALL 2026
As a condition of enrollment at Lafayette, the Bailey Health Center must ensure that students meet certain health requirements. It is critical that you allow sufficient time to see your healthcare provider.
All Bailey Health Center forms must be completed, records in any other format will result in a surcharge of $75 to the student account.
These forms are current for the 2026-2027 academic school year
All forms must be completed by July 15, 2026.
